Personal Care is a service offered to people who have medical conditions that limit their mobility and ability to perfor everyday tasks known as Activities of Daily Living (ADLs). Activities of Daily Living include:
Enrollment Process: The first step taken by Ability Group is to send a referral to your Primary Care Physician and a request for your last 3 notes from your physician visits, a list of the medications you currently are taking.
Once we receive the signed referral and the other required documentation we will schedule a time for a Registered Nurse to visit you in your home. The Registered Nurse will review your medical notes, ask you questions, check your vitals and observe your movements while performing tasks. Based off her observations a Plan of Care will be created that will be tailored to your specific needs.
The Plan of Care will be sent to your Primary Care Physician for approval. Once your Physician signs the Plan of Care a request of services will be sent to the insurance company. The insurance company has 20 business days to make a decision and will do one of the following:
It is your right to choose who you would like to take care of you! We understand that being cared for in your home requires trust with your caretaker and many of our clients choose to havea friend of family member care for them. For your friend or family member to begin work they will have to pass a background check and complete in-house training provided by Ability Group.
If you do not wish to have a friend or family member care for you, Ability Group will gladly find a Personal Care Worker to assist you.
Ability Group LLC accepts Title 19 without an HMO. If you are concerned your insurance will not cover Personal Care Services, please contact our offices and we will check your eligibility.
